Covina's own request record documents one bounded sidewalk-hazard report moving through the City's digital service pathway. The organization-authored record identifies Department: Streets, shows status Closed, gives a resolution date of August 11, 2026, and states that the trip hazards were ground down. This is direct evidence of what the City recorded and asserted, not contributor testimony about who performed the work. Current City documentation also identifies Streets as a Public Works division. The record does not identify the individual worker or crew, independently show the post-work physical condition, establish long-term durability, or support a City-wide performance conclusion.

What the City record shows

The request record identifies the issue as Sidewalk Repair, displays Department: Streets, shows the request as Closed, gives a resolution date of August 11, 2026, and contains the City-authored statement: “Trip hazards have been ground down. Thank you for reporting.”

Current City of Covina documentation separately describes Covina at your Service as routing submitted issues to responsible staff and providing status updates. The City's Public Works documentation identifies Streets as one of its divisions.

What this establishes

This is direct evidence of what the City recorded and asserted in one bounded request. It supports the observed administrative pathway: persistent request record, functional attribution to Streets, Closed status, dated resolution, and organization-authored remediation communication.

The contributor does not need to identify the individual person who operated equipment in order for UXR to preserve the City's own attribution and closure statement. If the City's record were inaccurate, that would become a record-accuracy or closure-fidelity issue for the system, not a failure of the contributor to provide eyewitness proof.

What remains separate

The screenshot does not independently demonstrate the physical post-work condition or long-term durability. It does not identify the individual worker or crew. The visible photo is not labeled as an intake or after-work image, so UXR does not infer which state it depicts. The submission age was displayed relatively rather than as a separately preserved absolute timestamp, so UXR does not derive a precise response interval from it.

Privacy

The raw request screenshot contains exact location and request-identifying information. The public UXR case summarizes the organization-authored evidence without publishing that raw image. Any future public image must redact exact location and other material re-identification details.

What changed in Public Revision 3

UXR corrected the evidence provenance. Earlier wording treated the field response and closure as contributor-reported or still awaiting direct screenshot review. The screenshot is itself a City-authored request record and directly supports the department, status, resolution date, and remediation wording. Independent physical outcome and durability remain separate questions.

The previously reviewed screenshot is itself a City-authored request record showing Department: Streets, Closed status, an August 11 resolution date, and the City's statement that the trip hazards were ground down. The contributor did not need to prove the City's own assertion. Remaining questions concern independent physical verification, durability, exact submission timing, and broader generalization.
