Respond to an HXR Evaluation or Incident

If you represent an organization discussed in an HXR record, you may correct facts, supply supporting or challenging evidence, explain constraints, offer an individual remedy, propose reform, document implementation, or ask HXR to verify a change. Responding does not require admitting fault.

Before you send the form

Your first contact is private. HXR does not publish an unverified submission, your identity, or your materials merely because you contacted us. We first verify authority, provenance, privacy, and what you permit us to attribute or publish.

Please use a work email address when practical. In the message field, include the Evaluation or Incident ID shown on the public record and your role or authority to respond for the organization.

Do not paste medical records, account numbers, customer data, confidential personnel information, privileged material, passwords, trade secrets, or other sensitive documents here. If supporting records are needed, HXR will arrange a more appropriate transfer method after authority is verified.

What happens after submission?

  1. HXR preserves the submission as received and creates a stable HXR-RESP response record.
  2. We acknowledge receipt and provide that reference.
  3. We verify that the respondent is authorized to speak for the organization or relevant unit.
  4. We separate factual correction, disagreement, evidence, remedy, reform, implementation, and verification requests rather than collapsing them together.
  5. Before anything from the response becomes public, HXR separately records attribution, quotation, evidence-publication, and redaction permissions.
  6. Supported corrections are reflected with dated, traceable revisions. Disagreement can remain visible without either side controlling the other's conclusions.

Important distinction: invitation versus formal HXR notice

Submitting this form does not create a deadline. HXR may invite an organization to respond without starting any public response clock. A governed HXR fair-notice period is a separate, explicitly authorized process that begins only after HXR verifies delivery of a specific notice.

About HXR

HXR is an independent public-interest publisher. It is not a regulator, court, law firm, certification body, or adjudicator. A response cannot buy deletion, favorable wording, a score, a badge, or control over HXR's evidence standards. Organizations receive visible credit for supported corrections, remedies, implementation, and verified reform when the evidence warrants it.
