Human Experience Reform public accountability record

Empire HVAC: restitution, reform, verification, and monitoring

One navigable home for two related Incidents and two reform Evaluations involving Empire Plumbing, Heating & Air Conditioning. Each record preserves its own scope and evidence while the shared restitution accounting is counted once.

One governed identity. Human Experience Reform (HXR) is the public publisher and governance identity. The UXR prefix survives only inside two stable Incident IDs so links and citations do not break. It is a record namespace, not a second project, organization, or methodology.

Shared restitution baseline

$1,765, counted once for the case family

$965 document-supported Empire expenditure + $800 contributor-reported independent corrective-service sequence = $1,765.

The $800 remains pending an independent invoice and task allocation. It reportedly covered a combined repair/recharge/maintenance sequence, so it is not assigned again to the maintenance Incident. The baseline is a transparent accounting anchor—not a finding that every line item was unnecessary, a legal damages figure, or $1,765 per page.

Additional maintenance-plan restitution is separate and unquantified. It depends on agreement terms, payment history, visit-level records, and a supported maintenance gap. Nothing on this page invents that amount.

What happened

An April 28, 2026 Empire invoice documents $965 in capacitor and refrigerant charges. The contributor reports having requested leak diagnosis and repair, later obtaining an independent corrective-service sequence reported at $800, and experiencing restored cooling. The contributor also reports a multi-year twice-yearly maintenance relationship and later being shown accumulated preventive-maintenance conditions.

What HXR evaluated

HXR separately evaluated (1) whether a leak-service pathway preserves a practical informed repair choice and (2) whether paid maintenance is defined, visit-level documented, auditable, and verifiable. Contributor accounts, primary artifacts, organization representations, and external context remain distinctly labeled.

What restitution is requested

Use the shared $1,765 baseline as the current accounting starting point, subject to evidence, correction, and agreement. Keep additional maintenance-plan restitution separate and unquantified until the missing records support it. A remedy can be offered without an admission.

Reform requested

Leak-service reform

Document the customer objective, appropriate diagnostics, findings and uncertainty, feasible options, recharge basis, material constraints and tradeoffs, and informed choice.

Maintenance reform

Define scope, use a system-specific checklist, record work and exceptions, preserve visit records, audit completion, surface accumulated conditions, and identify follow-up.

Acceptance tests

Leak-service test

A current low-refrigerant/suspected-leak service record must show the customer objective, appropriate diagnostics, uncertainty, feasible options with material tradeoffs, recharge basis, and documented informed choice. Current artifacts and observable repeatable practice are required.

Maintenance test

More than one current visit across a service cycle plus a recheck must show promised scope, system-specific checklist, completed work, observations, measurements, cleaning, exceptions, follow-up ownership, customer notice, and quality assurance.

Notice and organization response

  • Prior certified demand: contributor-reported; HXR has not reviewed the letter or verified-delivery artifact.
  • HXR organization notice: Not Yet Sent.
  • HXR response period: Not Started.
  • 30 calendar days: general HXR policy after verified HXR delivery, with one dated reminder; it is not currently running.

Neutral participation markers: Organization Responded and Evidence Supplied record process, not praise or blame. An offer and a published plan record proposals. Strong positive credit begins with completed remedy, contributor made whole, documented implementation, a passed acceptance test, or verified reform.

Empire may respond, correct, explain, propose a remedy, publish a reform plan, document implementation, or request verification without admitting fault. Authority and publication permissions are checked before any submission becomes public. Payment cannot buy deletion or favorable treatment.

What happens next

  1. Complete the missing independent-invoice/task-allocation and maintenance-record evidence where authorized.
  2. Keep private negotiation separate from HXR evidence review and public status.
  3. Send HXR notice only as a separate authorized action; start the general response clock only after verified delivery.
  4. Record remedy delivery separately from reform implementation, acceptance testing, verification, and monitoring.

What remains unresolved

  • Independent invoice and allocation of the reported $800 combined corrective-service sequence.
  • Which portion, if any, of the $965 Empire expenditure warrants restitution.
  • Maintenance-plan terms, payment history, visit records, supported gap, and any additional restitution.
  • Empire’s evidence, explanation, remedy position, or reform plan after verified HXR notice.
  • Whether current practice can pass both acceptance tests and sustain monitoring.

Packet, history, and institutional boundary

HXR uses append-only history: corrections, responses, remedy events, implementation evidence, test results, and reversals receive dated updates. HXR is not a regulator, court, law firm, certification body, or adjudicator. Organization outreach, formal demand, CSLB submission, and court filing are separate authorization-controlled actions.
